The Collaborative Care Center receives referrals from ÎåÔÂÌìÊÓƵ primary care physicians and select specialty physicians. The Collaborative Care Center’s patients receive consultations and evidence-based interventions to address common behavioral health concerns, such as anxiety and depression, adjustment-related issues, psychopharmacology in medical illness and pregnancy, and living with chronic disease. Patients prescribed medications at the Collaborative Care Center are transitioned back to their primary care physicians for ongoing management or referred out to longer-term psychiatric prescribers if appropriate.
